An alarming increase in the incidence of Gastrointestinal and liver diseases has led to MIOT Hospital opening a state – of- the – art MIOT Advanced Center for Gastro Intestinal and Liver Diseases. Established with an investment of more than 10 crores, this centre will offer comprehensive treatment and care for gastric and liver diseases unmatched in South India.

The incidence of Liver and GI Diseases in India
Gastrointestinal and liver diseases cause significant morbidity and mortality in our country. On rough estimate, 60-70% of our people suffer from disorders of the digestive system.

Diarrhoeal diseases, malabsorption and malnutrition lead to deaths or may result in physical and mental retardation. One in five child deaths in the world happen in India. A large proportion of these deaths are due to diarrhoeal diseases.

Epidemiological studies have shown that 4-7% of our population have chronic Hepatitis B Viral Infection. There are 350 million Hepatitis B virus infected people worldwide-42 million of them are in India. International data shows that approximately one million succumb to the infection every year.

Hepatitis B viral infection results in 10 times more deaths that AIDS in our country. The risk of death from Hepatitis B infection is often due to Cirrhosis or Hepatocellular Carcinoma. Hepatitis B infection can be prevented through immunization (vaccination) and can be treated effectively using antiviral drugs if identified early.

Hepatitis C Infection affects 1% of our people of the 11 million with this infection, the vast majority goes on to end stage liver diseases as the disease is detected very late. The spread of the disease is through improperly sterilized needles used for injections and unsafe blood transfusion.

Alcohol induced liver and pancreatic complications are on the increase. Studies report that 18-24% of adult males consume alcohol, many of them, in excess of recommended volumes. 8 million Indians are reported to have alcohol induced liver disease and 1.5 million suffer from cirrhosis of liver. This can lead to decompensated liver disease or cancer of the liver. Hepatocellur Carcinoma (Cancer of the liver) is the fourth most common cancer in the world. There are 14, 000 new patients of liver cancer every year.

Fatty liver is present in 20-30% of Indians. Non Alcoholic Fatty Liver Disease is a distinct clinical entity which is now known to lead to fibrous and later cirrhosis. As the prevalence of Diabetes and cardiac disease increases, there will be a corresponding increase in these affected with fatty liver as well.

Peptic Ulcer Disease has a point prevalence of 5% and a life time prevalence of 11%. It is known to result from the bacterium (Helicobacter Pylori). Gastro Esophageal Reflux Diseases that causes burning in the chest affects 8-5% Indians. The corresponding figures in the West are 10-20%. It has been shown that 60%of patients who report to emergency clinics actually suffer from Gastro Esophageal Reflux Disease and therefore have pain that is not cardiac in origin.

Oesophageal and stomach cancers are the third and fourth most common cancers in our country. Early detection and treatment reduces morbidity and mortality.

Acute and Chronic Pancreatitis results in severe abdominal pain and complications that necessitate management in an intensive care. Alcohol and Gall stone disease are among the causative factors. 10% of Indians suffer from gall stones.

Hepatitis A and E as well as Leptospirosis (Rat bite fever) can result in acute liver failure. These can be prevented through active public awareness, good hygiene and prevention strategies.

At the Centre, there are facilities for all the latest techniques in Therapeutic Endoscopy.

  1. The SPY Glass the first such installation in Tamil Nadu makes the MIOT Centre the third centre to have this facility in India. The SPY GLASS helps the Therapeutic Endoscopist to visualize the liver, biliary and pancreatic tree better, take biopsies when required, remove large stones and to perform intricate therapeutic procedures.
  2. CAPSULE ENDOSCOPY a non-invasive method to diagnose lesions in the small intestine makes it very convenient for the patient as he only has to swallow a capsule. Up to 10, 000 pictures are taken over 8 hours. The pictures are viewed by the Gastroenterologist to identify lesions .
  3. DOUBLE BALLOON ENTEROSCOPY is a later advance to the Single Balloon Enteroscopy which helps to visualize the small intestine and perform therapeutic procedures. This facility is also the first of its kind in the state.
  4. ERBE GI Work Station has facilities for Argon Plasma Coagulation and Thermal Therapy for hemostasis in tackling acute gastrointestinal bleeding. The Hybrid Knife uses a jet to raise the mucosa to help remove early cancers.
  5. ENDOSCOPIC ULTRASOUND combines digital Imaging as well as ultrasonography to diagnose and identify the extent of spread of tumors, to biopsy lesions and to proceed with therapeutic a solutions.
  6. PAEDIATRIC ERCP SCOPE the first in the state, the paediatric gastro scope and colonoscope will be part of the armamentarium for managing children’s gastrointestinal and liver disorders.
  7. NASAL GASTROSCOPE another first in the state provides patients with the facility to set up and have the endoscopy done without gagging or any other discomfort.
  8. NARROW BAND IMAGING using the 180 series provides the opportunity to visualize the epithelium and vascular lesions clearly and will help identify early cancer of the stomach.
  9. HIGH RESOLUTION MANOMETRY and PH monitoring will help in the diagnosis and management of mobility disorder and gastro esophageal reflux disease.
  10. INTERNATIONAL STANDARDS AND GUIDELINES for disinfection of endoscopes and accessories will be followed at the Centre to ensure total safety for patients and the endoscopists. A State-of-the-Art Reprocessing Centre with well designed storage facilities after thorough cleaning in an Olympus washer is another unique feature of the spacious MIOT CENTRE OF EXCELLENCE FOR ADVANCED GASTROINTESTINAL ENDOSCOPY.
  11. TRANSJUGULAR LIVER BIOPSY AND PRESSURE STUDIES in patients with advance liver disease and portal hypertension will be performed by Professor Murali and his team of Interventional Radiologists.
  12. Training Programmes In Advanced Therapeutic Endoscopy and a Dip NB Programme in Gastroenterology will ensure knowledge dissemination and will provide young Gastroenterologists with a facility to develop and further their skills in therapeutic endoscopy, to be of service to our people.

The MIOT Advanced Centre for Gastrointestinal and Liver Diseases will play a major role in establishing the MIOT Institute for Cancer Care. All diagnostic and therapeutic options for cancers relating to the gastrointestinal tract, the liver, the billary tract and pancreas will be made available for patients at a reasonable cost when the Institute opens.

Highly Professional and Skilled Team

Dr. George M. Chandy, MD, DM (Gastro), PGDHA, FRCP, FIHS, Formerly Professor and Head of Gastroenterology and Hepatology and Director, CMC, Hospital, Vellore will run the Liver Clinic and will set up the Liver Transplantation programme. He was trained at the Royal Prince Alfred Hospitals in Sydney, Australia and was one of the key team members who started the Liver Transplantation Programme at CMC, Vellore. He was responsible for setting up the Liver Unit in CMC, Vellore. The Government of India and the Medical Council of India honored him with the prestigious Dr.B.C. Roy National Award in the year 2006 in appreciation of his contribution to healthcare leadership in the country.

Dr. S. C. Samal, MD, DM (Gastro), formerly Associate Professor of Gastro enterology at CMC Hospital, Vellore. He is one of India’s best therapeutic Endoscopists and a well reputed Gastroenterologist.

Dr. Arul Prakash, MD (Paediatrics), DNB,DM (Gastro), formerly Asst. Professor of Gastroenterology, a caring, committed Paediatric Gastroenterologist who will be running the Paediatric Gastroenterology Unit and the Healthy Baby Clinic

Dr. Manoj Sahu, MD, DM (Gastro), formerly Asst. Professor of Gastroenterology at CMC Hospitals, Vellore, an efficient Gastroenterologist and skilled Endoscopist will look after patients a with gastrointestinal diseases.

The team will be ably supported by two excellent Gastrointestinal Surgeons Professor Dr. M. Subramanian and Dr. P. Kumar. Professor Subramanian was formerly Professor of Surgical Gastroenterology at SRMC, Porur and is well known for his surgical skills. Dr. P. Kumar did his Dip NB in Gastrointestinal Surgery at the Meenakshi Medical Mission and has already done several difficult surgeries using innovative techniques.

Professor Murali, a renowned Therapeutic Radiologist and an excellent team of Anaetheslologists headed by Professor Trevor Nair will be part of the Liver Transplantation Programme.
